Why do I need a support coordinator?
Navigating your NDIS plan can be tricky. Your support coordinator is there to help you find and connect with the right service providers to meet your needs and achieve your goals. This includes all types of support in your NDIS plan, such as informal, mainstream, community, and funded support all whilst ensuring you are informed, included and in control of the choices you decide so you can navigate your NDIS path with confidence.
Centacare’s Support coordinators will help you get the most out of your NDIS plan from start to finish.
We do this by helping you:
Understand your NDIS Plan – When you first receive your NDIS plan, it can be overwhelming. Your support coordinator will work with you to decode your plan and identify the supports that would be most beneficial to you.
Find Local Providers – With so many service providers available, sourcing the right one can be challenging. Our support coordinators will match you with providers that fit your needs, goals and budget and connect you with them.
Setup Service Agreements – Once you’ve chosen a service provider, your support coordinator will help set up a service agreement to ensure you receive quality care. Support Coordinators take a care of the finer details, from contacting the provider to keeping the agreement on file.
Providing Ongoing Guidance – As NDIS experts, our support coordinators offer advice and guidance throughout your NDIS journey. They are there to answer questions and resolve any issues that arise with providers.
As your support coordinator we:
Help you find:
- Plan managers
- Allied health services
Help you to:
- Understand and implement your NDIS plan
- Connect you with supports and services
- Establish and maintain these supports
- Plan and coordinate your support appointments
Help you by:
- Coordinating with LAC’s and other agency supports when an emergency or crisis arises.
Who is eligible for a Support Coordinator?
Unlike plan management, Support Coordination isn’t available to all NDIS participants. The NDIS funds it in certain cases, for example when:
- Participants are likely to build the skills and confidence needed to navigate the NDIS on their own. This is called capacity building, and it’s a key part of support coordination.
- Participants don’t have an informal support network, such as family or friends, who can help them find and connect with service providers. If you don’t have someone to provide this type of support, the NDIS might fund a support coordinator for you.
- Participants are likely to benefit significantly from having a support coordinator.
- Participants are new to the NDIS, receiving their first plan, or going through a major life change.
The NDIS reviews each participant individually to see if they’re eligible. If you request funding for support coordination in your planning meeting, the NDIS will consider your situation to decide if you qualify.

Support Coordinator
Step 1
Check your Capacity Building Budget - to see if Support Coordination been included in your Capacity Building budget plan? If Yes, got to step 2. If No, Ask your plan manager to apply for Support Coordination.
Step 2
Confirm what level of Support Coordination has been included in your plan. There are three* levels of support coordination:
- Support connection – a support to build your ability to connect with informal, community and funded supports enabling you to get the most out of your plan and pursue your goals.
- Support coordination – coordination of supports: a support to assist you to build the skills you need to understand and use your plan. A support coordinator will work with you to ensure a mix of supports are used to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
- Specialist support coordination – a higher level of support coordination. It is for people whose situations are more complex and who need specialist support. A specialist support coordinator will assist you to manage challenges in your support environment and ensuring consistent delivery of service.
* As of August 2024, Centacare offers level 2, Support Coordination.
