Eligibility
Who is eligible for Support Coordination?
Support Coordination funding is approved by the NDIS for people who would benefit from expert guidance in managing their plan. You may be eligible if:
- You are new to the NDIS or have recently experienced major life changes.
- You don’t have a strong informal support network to assist you.
- You’d benefit from help connecting to and coordinating your supports.
- You’re building the skills to manage your plan independently in future.

How to engage us
Getting started is simple
- Check your plan – Look for Support Coordination funding under your Capacity Building budget.
- Confirm your level – Centacare offers Level 2 Support Coordination, helping you strengthen your ability to manage supports and live independently.
- Submit a Request for Service (RFS) – Log in to proda.humanservices.gov.au or contact us directly on 1300 619 379 to lodge your request.
